Abstract

We have studied 176 men, including 84 with acute myocardial infarction (mean age 59,2±2,2), 92 (mean age 49,2±3,5) with angina of various functional classes. We have registered 12-lead ECG at rest (50 mm/sec) to all simultaneously. QT and QTc dispersion were calculated as the difference between their maximal and minimal durations in all the 12 leads. The prognostic role of QT dispersion as a marker of potentially life-threatening arrhythmias, is insufficient in the angina group. Unlike those, patient with acute myocardial infarction demonstrate a far greater role QT time parameters in predicting potentially life-threatening arrhythmias; with QT dispersion sensitivity and specificity (threshold > 50 msec) proved 82,2% and 85,8% respectively, and for QTc (threshold ≥ 70 msec) – 88,1% and 94,5%.

Abstract

In 97 patients with acute miocardium infarction have been studied hemoreological parameters, in 20 patients have been defined elastase blood activity and of A1 AT level. 67 patients were given laser therapy. Results: laserotherapy of low intensivity in patients with miocardium infarction reduces the frequency of postinfarction angina blood deficiency, heart rythm breaks development, decreases a number of miocardium infarction recidivs and mortality. Was noted the reduce of thrombocytes aggregation (23%, p< 0,01), GDF level (18%, p< 0,01) increasing of antitrombin III quantity (18%, p<0,01) prolongation TV and ACHTV. Elastase blood activity in IM patients exceeded 2 time of normal parameters. Therapy by laser didn’t effect on this parameter, but significantly increased of A1 AT level, that is main inhibitor of leucocyte’s elastase. Reduce of elastase - A1 AT complex on background of laserotherapy was correlated with improvement of gemoreological blood properties.

Abstract

The aim of this research was to study the clinical meaning of dynamic of blood serum mioglobine level in hypertensives with episodes of painless ishemia of hypertrophied miocardium 128 patients (23 women among them) with hypertension desease II stage (according to WHO classification, 1993) aged between 35 and 58 years (average age 25,3±1,2 year) were treated by through-oesophagus stimulation of auricles, ECG daily monitoring was conducted, 24-hours profile of serum Mg was studied. Nature of structural and functional miocardium changes and their correlation with frequency of painless ishemia of myocardium was analized. Results:duration of painless depression of ST segment is proportionally and significantly correlated with MMLV (r=0,88 p<0,01). Group of GD II stage with high level serum Mg after TOSA-test significantly differed both in the quantity of episodes of painless depression of ST segment (12,5±2,69 against 5,66±0,86 , p<0,002) and for their duration (4,1±1,02 against 2,45±0,41 min, correspondingly, p<0,001). Clinical moment of Mg level increase in GD II stage patients was defined.

Abstract

In this scientific work we study of myocardial scintigraphy with the new russian radionuclide 99mTc-technetril to diagnosis of ischemic heart disease. We observed 98 patients (83 male) aged 55±5 years. Our data showed the correlation between exercise electrocardiography (bicycle exercise test) and disorders of myocardial perfusion (myocardial scintigraphy). It was demonstrated that exercise scintigraphy more informational in diagnostics lesion of ischemic heart disease in patients group with negative stress-test

Abstract

The aim of this research was to study the functional coronary reserve and frequency of miocardium infarction in patients with cardiologic syndrome X (CX) in comparison with patients with stenosing atherosclerosis of coronary artery (ACA). 54 patients with CX and 83 patients with ACA were examined. Coronaroventriculography,veloergometry, echo, daily ECG monitoring were conducted. It was proved that functional coronary reserve in CX patients is lower than in ACA patients. However risk of miocardial infarction development is higher in ACA patients. In the first place, functional coronary reserve depends on state of terminal sectors of coronary arterial channel both in CX and ACA patients. At the same time risk of miocardial infarction development is above all conditioned by damage of subepicardial arteries, i.e. presence of atheromas in them and relatively little depends on volume of coronary channel’s damage.

Abstract

48 patients (all men) hospitalized on account of CCI stages II B stage by Strazhesko-Vasilenko were examined. Clinical effect of trimetazidine in dosage of 60 mg daily was investigated. It was proved that inclusion of the medicine in complex therapy of coronary deficiency contributes to improvement of miocardium inothrope function. These positive changes have to do with trimetazidine’s positive influence on lipid peroxidation and blood reology in examined patients.

Abstract

A task of this research was to study the influence of nebivolol on hemodynamic parameters, clinical sympthoms and life quality in patients II-IV FK HD by NYHA. 24 patients of main group have taken nebivolol in daily dose of 2,5-5 mg per os. Patients were treated with the drug in a course of 3 months.A control group(15 people) was treated traditional therapy (by heart glicozides, metabolic preparations, diurethics, if necessary). We have found that new β-adrenoblocker with additional vasodilatatorial qualities- nebivolol (NEBILET) kindly influences main hemodinamic parameters in chronic coronary patients. During 3 months’ observation examined patients had KDR and KSR distinctly lowered, speed of shortening of miocardium fibrem and the function of throw and MV grew, tension in a pulmonary artery decreased. NEBILET’S prescription in dosage of 2,5-5 mg/ daily kindly influences the reduction of heart rythm’s disturbances, a distinct tendency to lowering of MMLV noted. NEBILET can be successfully used in a combinated therapy of CCD II-IV FK, its usage in cases of coronary deficiency and AG is mostly advisable.

Abstract

We have examined fourty-nine patient with an acute disturbance of cerebral circulation (ADCC). Holter ECG monitoring revealed 43% of cases having some episodes of myocardial ischemia. In ADCC the control subjects received complex therapy, the experimental group subjects being given emoxipin in addition. The use of emoxipin was seen to reduce the incidence of myocardial ischemia and to product a positive effect on its structure.

Abstract

Metabolic effects of moxsonidine (cynt) were examined in 15 patients with mild and moderate arterial hypertension (blood-pressure within 140-179/90-109 mm Hg, according to recommendations of WHO and WOH ,1999) combining with compensated diabetes of type II. After 3 month of treatment with the drug (titration of dosage from 0,2 mg to 0,6 mg) significant lowering of insuline and glucose levels in blood happened; level tests were performed two hours after standard breakfast (an equivalent of the tolerance test to glucose). Average level of glucose was 9,28±0,62 mM/l before treatment and 8,44±0,51 mM/l after treatment (p<0,04). Indicated results show an improvement of tissue sensitivity to insuline as there is need in less insuline in need after treatment with cynt for maintenance of lowered as before treatment level of glucose. Besides, growth of cholesterol content in lipoproteids of high density occured as the result of cynt treatment of patients. A conclusion about favourable metabolic effects of cint and advisability of its usage as antihypertension drug for patients with AG and diabetes of type II was drawn.

Abstract

Peculiarities of programmed through-oesophagus electrocardiostimulation (PTOECS) in 84 coronary patients and 35 healthy have been studied with the purpose of exposure of new additional criterions of lowering of coronary reserve (CR). It was discovered that 22,8% of healthy people and 38,1% patients with coronary patology by PTOECS had signs of blocades of fasciculus atrioventricularis (BFAV) at the moment of achievement of reflexity of miocardium, which were present on the background of lowered CR in cases of patology. Healthy people had the presence of this phenomenon more often in ombination with high level of conductivity in atrioventricular (AV) combination by that. On the contrary, coronary patients with BFAV had blocades of conducting accompanied by essential prolongation of effective refractering period, compared to analogue subgroup of healthy persons (correspondingly, 258,6±15,4 and 232,1±12,8 mls?; p<0,01). Time of preservation of BFAV (period of ”vulnerability”)appeared to be significantly longer in patients, compared to healthy people (correspondingly, 91,8±8,3 and 18,4±2,1ms; p<0.001). Significant prolongation of a period of “vulnerability “ was noted at the same time with the grouth of the dangerousness of illness; combination of BFAV with hidden disturbance of conductivity in AV combination was noted in case of appearance of ishemic threshold on low frequencies of stimulation. The conclusion about possibility of using of parameters studied by PTOECS for specification of presence and grade of dangerousness of CR, was drawn.

Abstract

Experimental data that have appeared lately are the evidence of the fact that sympathetic nerve exerts constant (tonic) stimulating influence on heart activity, what contradicts to formed (classical) conception on regulation of heart activity by a sympathetic nerve. However experimental data us have worked under conditions of chronical experiment with blocade of sympathetic and parasympathetic nerve system by pharmaceutical medications, on the contrary, confirm classical ideas of the abscence of sympathetic nerve’s tonic influence on heart. According to our data, decrease of frequency of heart contractions in other specialists’ experiments is a result of effects of experiment’s conditions (narcosis’ kind and depth, possible hypothermia, experiment’s duration, sort of pharmaceutical medications). Frequency of heart contractions, as distinct hau the of blood vessels, is assessed in rest by a level of expression of wandering n.vagus tone and humoral substances circulating in blood. Classical concepts of abscence of tone of sympathetic nerves for heart remain in effect.
