Роль полиморфизма и особенностей экспрессии генов рецепторов врожденного иммунного ответа в патогенезе инфекционного эндокардита

Аннотация

Инфекционный эндокардит (ИЭ) — заболевание, как правило, бактериального генеза, смертность от которого находится на четвертом месте среди других летальных инфекционных заболеваний, уступая сепсису, пневмонии и внутрибрюшному абсцессу. В последние годы наблюдается тенденция к увеличению случаев протезного ИЭ. Для ИЭ актуальна проблема антибиотикорезистентности, приводящей к снижению эффективности его терапии. Вопрос разработки новых эффективных методов профилактики данного заболевания, а также оценка тяжести его последствий представляется чрезвычайно актуальным, особенно в контексте развития персонифицированной медицины. Известно, что иммунный ответ играет важную роль в патогенезе ИЭ, при этом большое значение в развитии и течении данного заболевания имеют рецепторы врожденного иммунного ответа. Полиморфизм генов, кодирующих данные рецепторы, приводит к изменению их функциональной активности, что обуславливает эффективность реакции организма на инфекцию. В данном обзоре проанализировано влияние полиморфизма ряда ключевых генов рецепторов врожденного иммунного ответа, а также изменение их экспрессии у пациентов с ИЭ, и показана их значимость в формировании индивидуальной чувствительности пациентов к развитию данной патологии.
