Практические аспекты антитромботической терапии у пациентов с фибрилляцией предсердий и ишемической болезнью сердца

Аннотация
Фибрилляция предсердий (ФП) является наиболее распространенной аритмией сердца и связана с повышенным риском сердечно-сосудистой смерти, при этом последнее обстоятельство частично объясняется нередким сочетанием с ишемической болезнью сердца (ИБС). При наличии показаний пациентам с ФП необходима постоянная терапия оральными антикоагулянтами для предотвращения тромбоэмболических осложнений. Кроме того, пациентам с ИБС, которым необходимо выполнение неотложного или элективного чрескожного коронарного вмешательства, требуется назначение двойной антитромбоцитарной терапии. Поэтому у врачей часто возникает серьезная дилемма при выборе наиболее адекватного режима антитромботической терапии для пациентов с ФП, подвергающихся чрескожным коронарным вмешательствам. Интегрирование двух медикаментозных подходов к лечению одного пациента с сопутствующими ФП и ИБС является сложной задачей и должно соблюдать баланс между высокой степенью эффективности и безопасности. В статье приводится обзор исследований, посвященных этой проблеме.
