изучить связь морфометрических характеристик адипоцитов эпикардиальной жировой ткани (ЭЖТ) со степенью её накопления, антропометрическими показателями ожирения и особенностями метаболических нарушений у пациентов с ИБС, подвергнутых аортокоронарному шунтированию (АКШ), в зависимости от наличия или отсутствия сахарного диабета типа 2 (СД2).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Включены 46 пациентов (м/ж 38/8), 44–75 лет с ИБС, имеющих показания для АКШ. Адипоциты ЭЖТ получали интраоперационно, определяли их размер и степень гипертрофии. Объем ЭЖТ оценивали с помощью бесконтрастной компьютерной томографии сердца, толщину ЭЖТ - по данным эхокардиографии. Изучали липидтранспортную функцию крови, углеводный обмен, уровни пропротеиновой конвертазы субтилизин/кексин типа 9 (PCSK9) и адипокинов.</p></sec><sec><title>Результаты</title><p>Результаты. В общей группе средний размер адипоцитов коррелировал с отношением адипонектин/лептин (rs=-0,49, p=0,001), уровнем С-пептида (rs=0,47, p=0,002), PSCK9 (rs=0,45, p=0,004), триглицеридов (rs=0,39, p=0,014) и индексированным на поверхность тела объемом ЭЖТ (rs=0,36, p=0,022). При наличии СД2 (n=14) средний размер адипоцитов коррелировал с весом (rs=-0,72, p=0,013), окружностью талии (rs=-0,64, p=0,033), бедер (rs=-0,62, p=0,042) и уровнем триглицеридов (rs=0,60, p=0,043). У пациентов без СД2 (n=32) эти корреляции отсутствовали, а размер адипоцитов демонстрировал корреляционные связи с объемом ЭЖТ (rs=0,41, p=0,025), уровнем в крови PCSK9 (rs=0,41, p=0,025), лептина (rs=0,42, p=0,023) и отношением адипонектин/лептин (rs=-0,46, p=0,012). В обеих группах толщина ЭЖТ не имела корреляционных связей с ее объемом. Согласно модели логистической регрессии вероятность выявления крупных и гипертрофированных адипоцитов ЭЖТ определяется уровнем в крови PCSK9, объемом ЭЖТ и наличием СД2. Прогностическая точность модели 90%, чувствительность 85,71%, специфичность 94,74%.</p></sec><sec><title>Заключение</title><p>Заключение. У пациентов с ИБС и коронарным атеросклерозом наличие СД2, уровень PCSK9 и объем ЭЖТ в комбинации определяют вероятность выявления гипертрофии адипоцитов ЭЖТ. Прямая связь между размером адипоцитов ЭЖТ, степенью его гипертрофии и объемом ЭЖТ установлена только для пациентов без СД2. Для корректной оценки степени аккумуляции ЭЖТ целесообразно оценивать ее объем.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To investigate the associations of morphometric characteristics of epicardial adipose tissue (EAT) adipocytes with EAT accumulation, anthropometric obesity parameters, and metabolic disturbances in patients with coronary artery disease (CAD) undergoing coronary artery bypass grafting (CABG), depending on the presence or absence of type 2 diabetes mellitus (T2DM).</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 46 patients (38 men, 8 women) aged 44–75 years with CAD undergoing CABG. EAT adipocytes were obtained intraoperatively, and their size and hypertrophy degree were assessed. EAT volume was measured using non-contrast cardiac computed tomography, while EAT thickness was evaluated by echocardiography. Lipid profile, carbohydrate metabolism parameters, proprotein convertase subtilisin/kexin type 9 (PCSK9) levels, and adipokine concentrations were determined.</p></sec><sec><title>Results</title><p>Results. In the overall cohort, mean EAT adipocytes’ size showed correlations with the adiponectin/leptin ratio (rs=-0.47, p=0.001), C-peptide (rs=0.47, p=0.002), PCSK9 (rs=0.45, p=0.004), triglycerides (rs=0.39, p=0.014), and body surface area-indexed EAT volume (rs=0.36, p=0.022). In T2DM patients (n=14), mean adipocytes’ size negatively correlated with body weight (rs=-0.72, p=0.013), waist circumference (rs=-0.64, p=0.033), and hip circumference (rs=-0.62, p=0.042) and with triglyceride levels (rs=0.60, p=0.043). In non-diabetic patients (n=32), these associations were absent, but mean adipocytes’ size correlated with EAT volume (rs=0.41, p=0.025), with PCSK9 (rs=0.41, p=0.025) and leptin (rs=0.42, p=0.023) levels, as well as with the adiponectin/leptin ratio (rs=-0.46, p=0.012). In both groups, the EAT thickness had no correlation with its volume. A logistic regression model was constructed, demonstrating that the probability of detecting large and hypertrophic EAT adipocytes is determined by PCSK9 levels, EAT volume, and the presence of T2DM. The model demonstrated 90% predictive accuracy, 85.71% sensitivity, and 94.74% specificity.</p></sec><sec><title>Conclusion</title><p>Conclusion. In patients with CAD, the presence of T2DM, PCSK9 levels and EAT volume in combination accurately predict the likelihood of detecting EAT adipocyte hypertrophy. A direct relationship between EAT adipocyte size, the degree of hypertrophy, and EAT volume has only been established for patients without T2DM.
