Оценить показатели системного воспалительного ответа при миокардите у пациентов с фибрилляцией предсердий.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследуемую выборку включено 63 пациента, которые проходили интервенционное лечение фибрилляции предсердий в стационаре. Из них 49 (77,8%) мужчин. Медиана возраста составила 46 (39; 54) лет. Во время катетерной процедуры всем больным проведена эндомиокардиальная биопсия с последующей гистологической диагностикой. Ретроспективно оценена роль показателей системного воспаления у больных с миокардитом и без.</p></sec><sec><title>Результаты</title><p>Результаты. Хронический миокардит выявлен у 30 (46,1%) больных. "Индекс системного воспалительного ответа" (p=0,04), "совокупный системный индекс воспаления" (p=0,02) и "моноцито-лимфоцитарное соотношение" (p=0,02) достоверно были выше в группе без миокардита. Проанализированы ROC кривые данных показателей. Множественный регрессионный анализ не показал прогностического влияния факторов на наличие миокардита.</p></sec><sec><title>Заключение</title><p>Заключение. Показатели "индекс системного воспалительного ответа", "совокупный системный индекс воспаления" и "моноцито-лимфоцитарное соотношение" достоверно были выше в группе пациентов без миокардита. "Моноцито-лимфоцитарное соотношение" было больше у больных с 2 и выше стадией фиброза миокарда. "Индекс системного воспалительного ответа" и "моноцито-лимфоцитарное соотношение" значимо ниже у больных с более тяжелой симптоматикой по классификации EHRA.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate parameters of the systemic inflammatory response in myocarditis in patients with atrial fibrillation.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study sample included 63 patients undergoing inpatient interventional treatment for atrial fibrillation. Of these, 49 (77,8%) were men. The median age was 46 (39; 54) years. During the catheter procedure, all patients underwent endomyocardial biopsy, followed by histological diagnosis. The role of systemic inflammation parameters in patients with and without myocarditis was retrospectively assessed.</p></sec><sec><title>Results</title><p>Results. Chronic myocarditis was detected in 30 patients (46,1%). The systemic inflammatory response index (SIRI) (p=0,04), total systemic inflammation index (SII) (p=0,02), and monocyte-to-lymphocyte ratio (MLR) (p=0,02) were significantly higher in the group without myocarditis. ROC curves for these parameters were analyzed. Multiple regression analysis revealed no predictive effect of these factors on myocarditis.</p></sec><sec><title>Conclusion</title><p>Conclusion. The SIRI, SII, and MLR values were significantly higher in patients without myocarditis. The MLR was higher in patients with stage 2 and higher myocardial fibrosis.
