Оценить частоту, клинические особенности и прогностическое значение сахарного диабета 2 типа (СД2) у амбулаторных пациентов с сердечной недостаточностью (СН).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. По данным исследования ПРИОРИТЕТ-ХСН в когорте 19939 пациентов с СН проведён ретроспективный анализ частоты СД2, особенностей контроля и терапии данных коморбидных состояний. Методом многофакторной регрессионной модели Кокса оценена прогностическая роль СД2 и гликированного гемоглобина (HbA1c).</p></sec><sec><title>Результаты</title><p>Результаты. Частота СД2 у пациентов с СН составила 27,4%. Уровень HbA1c на Визите 1 был определён у 37,6% пациентов с СД2 и в 11,6% случаев при отсутствии СД. Пациенты с СД2 по сравнению с группой без СД2 характеризовались большим бременем сердечно-сосудистых коморбидных заболеваний и таких некардиальных сопутствующих состояний, как хроническая болезнь почек, анемия, бронхиальная астма. У пациентов с СД2 отмечались более высокий функциональный класс, большая длительность СН, более частые госпитализации по поводу СН в анамнезе. Исходно наличие СД2 ассоциировалось с более частым назначением блокаторов рецепторов ангиотензина II, блокаторов ренин-ангиотензин-альдостероновой системы, антагонистов минералокортикоидных рецепторов, бета-блокаторов, ингибиторов натрий-глюкозного котранспортёра 2 типа (иНГТ2). В динамике после Визита 1 в обеих подгруппах отмечено увеличение назначения всех классов рекомендованной болезнь-модифицирующей терапии (РБМТ), особенно иНГТ2. Доли пациентов, получающих целевые дозы РБМТ, были низкими в обеих подгруппах с и без СД2, однако титрование доз всех классов РБМТ было более эффективным среди пациентов с СД2. Для контроля гликемии чаще использовался метформин, вторую позицию занимали иНГТ2 и третью — препараты сульфонилмочевины, не являющиеся препаратами выбора при СН. Пациенты с СД2 характеризовались более неблагоприятным прогнозом по сравнению с когортой без СД2. В подгруппе пациентов с СД2 уровень HbA1c &gt;9% был ассоциирован с увеличением вероятности смерти от всех причин, а также госпитализаций по любым причинам и по поводу СН.</p></sec><sec><title>Заключение</title><p>Заключение. Наличие СД2 у пациентов с СН ассоциировано с более тяжёлым киническим течением СН, ухудшает прогноз и требует повышения осведомлённости врачей по диагностической и лечебной тактике ведения данной когорты пациентов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To assess the prevalence, clinical features and prognostic value of type 2 diabetes (T2D) in outpatients with heart failure (HF).</p></sec><sec><title>Material and methods</title><p>Material and methods. According to the PRIORITY-HF study, a retrospective analysis of T2D prevalence, features of control and treatment of related comorbid conditions was performed in a cohort of 19939 patients with HF. The prognostic role of T2D and glycated</p><p>hemoglobin (HbA1c) was estimated using the multivariate Cox regression model. Results. The prevalence of T2D in patients with HF was 27,4%. The HbA1c level at visit 1 was determined in 37,6% of patients with T2D and in 11,6% of cases without T2D. Patients with T2D compared to those without T2D were characterized by a higher burden of cardiovascular comorbidities and such non-cardiac concomitant conditions as chronic kidney disease, anemia, bronchial asthma. Patients with T2D had a higher functional class, longer duration of HF, and a greater HF-related hospitalization rate. At baseline, T2D was associated with more frequent prescription of angiotensin II receptor blockers, renin-angiotensin-aldosterone system blockers, mineralocorticoid receptor antagonists, beta-blockers, sodium-glucose cotransporter-2 (SGLT2) inhibitors. After visit 1, both subgroups showed a prescription rate increase in all classes of guideline-directed medical therapy (GDMT), especially SGLT2 inhibitors. The proportions of patients receiving target GDMT doses were low in both subgroups with and without T2D, but titration of doses of all GDMT classes was more effective among patients with T2D. Metformin was most often used for glycemic control, followed by SGLT2 inhibitors and sulfonylureas, which are not front-line therapy for HF. Patients with T2D were characterized by a more unfavorable prognosis compared to the cohort without T2D. In the subgroup of patients with T2D, HbA1c &gt;9% was associated with an increased all-cause mortality, as well as all-cause and HF-related hospitalization rates.</p></sec><sec><title>Conclusion</title><p>Conclusion.
