Оценить глобальную деформацию левого желудочка (ЛЖ) в продольном направлении после стентирования коронарных артерий (КА) у больных стабильной ишемической болезнью сердца (ИБС).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Включено 50 больных стабильной ИБС (35 мужчин, 15 женщин, возраст — 59,3±6,89 лет). По показаниям всем пациентам проведено стентирование КА. Syntax Score не превышал 22 усл. ед. (11,78±6,77 усл. ед.). До и в течение первой недели после стентирования КА всем пациентам выполнена стандартная эхокардиография (ЭхоКГ), оценена глобальная деформация ЛЖ в продольном направлении (Global Longitudinal Strain-GLSLV) c использованием опции “AFI”. Всем пациентам проводилась количественная оценка тропонина I и МВ-фракции креатинфосфокиназы (КФК МВ) до вмешательства, через 6 ч и 24 ч после стентирования.</p></sec><sec><title>Результаты</title><p>Результаты. До стентирования КА у 21 пациента отмечалась GLSLV в пределах нормы (-18,0% и менее), у 29 больных наблюдалось снижение GLSLV (более -18%). После стентирования КА выявлены пациенты с улучшением GLSLV (n=28) и с ухудшением GLSLV (n=22). В раннем послеоперационном периоде не наблюдалось клинически значимых осложнений, электрокардиографических изменений, усугубления нарушений локальной сократимости. В группе ухудшения GLSLV обнаружено повышение уровней тропонина I и КФК МВ через 6 ч и 24 ч после стентирования, не превышающее пороговое значение для острого коронарного синдрома (ОКС).</p></sec><sec><title>Заключение</title><p>Заключение. Ухудшение глобальной деформации ЛЖ в продольном направлении после стентирования КА наблюдалось у 44% больных стабильной ИБС. Отрицательная динамика GLSLV ассоциирована с повышением уровней тропонина I, КФК МВ через 6 ч и 24 ч после стентирования КА, не превышающим пороговое значение для ОКС. Ухудшение глобальной деформации ЛЖ и повышение уровня тропонина I, вероятно, обусловлены эмболией микрососудистого русла во время проведения стентирования КА. Повреждение миокарда во время стентирования КА обусловливает дальнейшее снижение деформации у пациентов с исходно сниженной деформацией.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate the global longitudinal strain of the left ventricle (LV) after coronary stenting (CS) in patients with stable ischemic heart disease (IHD).</p></sec><sec><title>Material and methods</title><p>Material and methods. Totally, 50 IHD patients included (35 men, 15 women, age 59,3±6,89 y.). All had indications for CS. Syntax Score was not higher than 22 units (11,78±6,77 units). Before and during the first week after CS all patients underwent standard echocardiography (EchoCG), where the Global Longitudinal Strain-GLSLV was assessed with “AFI” option. All patients also got quantitative measurement of troponin I and MB-creatinkinase (CK-MB) before intervention, in 6 hours and in 24 hours after stenting.</p></sec><sec><title>Results</title><p>Results. Before CS, in 21 patient there was normal GLSLV (-18,0% and less), in 29 GLSLV decreased (more than -18%). After stenting the patients with improvement of GLSLV were marked (n=28) and with worsening of GLSLV (n=22). During early postoperation period there were no any clinically significant complications, electrocardiographic changes, worsenings of local contractility. In worsening of GLSLV group there was increase of troponin I and CK MB in 6 hours and in 24 hours after stenting, not more than threshold value of acute coronary syndrome (ACS).</p></sec><sec><title>Conclusion</title><p>Conclusion. Worsening of global longitudinal strain of the LV after CA stenting was found in 44% of stable IHD patients. Negative dynamics of GLSLV was associated with an increase of troponin I, CK-MB levels in 6 and 24 hours after CS not higher significant for ACS level. Worsening of global strain of the LV and increase of troponin, probably, can be explained by embolism of microcirculatory pool during CA stenting.
