Оценить влияние момента измерения артериального давления (АД) и положения тела пациента при этом на значения АД и показатели миокардиальной работы сердца при проведении эхокардиографии (ЭхоКГ).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В проспективное исследование включено 100 пациентов в возрасте от 21 года до 84 лет. АД измерялось на правой руке с помощью механического тонометра 4 раза в зависимости от времени и положения тела пациента: перед ЭхоКГ в положении пациента на спине (АД1); перед ЭхоКГ в положении пациента на левом боку (АД2); во время ЭхоКГ в момент выведения апикальных позиций (АД3); после завершения ЭхоКГ, когда пациент снова лежал на спине (АД4). Для расчета миокардиальной работы использовали методику построения кривых "давление-деформация" с каждым из полученных измерений АД.</p></sec><sec><title>Результаты</title><p>Результаты. Наиболее высокое систолическое АД (САД) было при исходном измерении (АД1) и составляло 120±12 мм рт.ст. Между АД1 и АД2 измерениями САД значительно снизилось (р&lt;0,001). Снижение САД между АД2 и АД3 составило 3% (р=0,598). При измерении АД3 САД снизилось на 10,3% от исходного и было самое низкое 109±12 мм рт.ст. Между измерениями АД3 и АД4 значения САД значительно возросли (р&lt;0,005). Показатели САД при АД4 были значительно ниже по сравнению с АД1 (р&lt;0,001). Соответственно индекс миокардиальной работы составил 1716±301 мм рт.ст.% при АД1, снизился до 1597±277 мм рт.ст.% и 1551±260 мм рт.ст.% при АД2 и АД3, соответственно, и увеличился до 1623±254 мм рт.ст.% при АД4 (рАД1-АД2&lt;0,005; рАД2-АД3=0,226; рАД3-АД4&lt;0,05; рАД1-АД4&lt;0,05).</p></sec><sec><title>Заключение</title><p>Заключение. Для получения достоверных и сопоставимых показателей при расчете миокардиальной работы необходим стандартизированный подход к измерению АД. Стандартизированное измерение АД подразумевает выполнение его в определенный момент времени и при определенном положении тела пациента. Оптимальные время и положение пациента при измерении АД для расчета миокардиальной работы — в конце ЭхоКГ исследования, после поворота пациента на спину.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate the impact of the blood pressure (BP) measurement timing and the patient’s positioning on BP values and myocardial performance data according to echocardiography.</p></sec><sec><title>Material and methods</title><p>Material and methods. This prospective study included 100 patients aged 21 to 84 years. BP was measured on the right arm using a mechanical BP monitor four times depending on the time and patient positioning as follows: before echocardiography in the supine position (BP1); before echocardiography in the left lateral position (BP2); during echocardiography when apical view were established (BP3); after echocardiography, when the patient was again lying supine (BP4). To calculate myocardial work, the pressure-strain curve method was used for each of the obtained BP measurements.</p></sec><sec><title>Results</title><p>Results. The highest systolic blood pressure (SBP) was observed during the baseline measurement (BP1) and amounted to 120±12 mm Hg. Between BP1 and BP2 measurements, SBP significantly decreased (p&lt;0,001). The decrease in SBP between BP2 and BP3 was 3% (p=0,598). During BP3 measurement, SBP decreased by 10,3% from the baseline (lowest, 109±12 mm Hg). Between BP3 and BP4 measurements, SBP values increased significantly (p&lt;0,005). The SBP values at BP4 were significantly lower compared to BP1 (p&lt;0,001). Accordingly, the myocardial work index was 1716±301 mm Hg% at BP1, decreased to 1597±277 mm Hg% and 1551±260 mm Hg% at BP2 and BP3, respectively, and increased to 1623±254 mm Hg% at BP4 (pBP1-BP2&lt;0,005; pBP2-BP3=0,226; pBP3-BP4&lt;0,05; pBP1-BP4&lt;0,05).</p></sec><sec><title>Conclusion</title><p>Conclusion. To obtain reliable and comparable values when calculating myocardial work, a standardized approach to blood pressure measurement is necessary. Standardized blood pressure measurement implies its performance at a specific point in time and in a specific body position of the patient.
