Оценить кальцификацию грудного отдела аорты и параметры суточного мониторирования артериального давления (СМАД) у пациентов с резистентной артериальной гипертензией (РАГ).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включены 138 пациентов с диагнозом РАГ. По результатам СМАД (Schiller BR-102 plus) пациенты разделены на группы контролируемой (1 группа, n=68) и неконтролируемой РАГ (2 группа, n=70). Кальциевый индекс (КИ) грудного отдела аорты определен с помощью 64-спирального компьютерного томографа ("Siemens", Германия) в процессе сканирования от корня до диафрагмы.</p></sec><sec><title>Результаты</title><p>Результаты. Статистически значимых отличий между группами по основным клинико-антропометрическим характеристикам не выявлено. КИ составил от 312 до 2738 единиц Агатстона и оказался выше в группе неконтролируемой РАГ (р&lt;0,00001). С помощью ROC анализа установлено, что КИ величиной 900 ед. Агатстона с чувствительностью 71,4% и специфичностью 66,2% отличает лиц с контролируемой и неконтролируемой РАГ (AUC=0,791). При анализе результатов СМАД наиболее значимые отличия получены при сопоставлении пульсового артериального давления (ПАД), суточного индекса систолического артериального давления (САД), величины утреннего подъема и вариабельности САД в дневные часы. Независимо от эффективности антигипертензивной терапии КИ положительно коррелировал с вариабельностью САД, ПАД, величиной утреннего подъема и отрицательно с суточным индексом САД, но сила связи больше при недостижении целевого уровня АД.</p></sec><sec><title>Заключение</title><p>Заключение. У пациентов с РАГ при отсутствии контроля АД отмечаются более высокие значения ПАД, величины утреннего подъема и вариабельности САД, снижение суточного индекса САД. КИ выше в группе пациентов с неконтролируемой РАГ и связан с вариабельностью САД, ПАД, величиной утреннего подъема АД и степенью ночного снижения САД.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate thoracic aortic calcification and parameters of 24-hour ambulatory blood pressure monitoring (ABPM) in patients with resistant hypertension (HTN).</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 138 patients diagnosed with resistant HTN. Based on ABPM (Schiller BR-102 plus), the patients were divided into groups of controlled (Group 1, n=68) and uncontrolled resistant HTN (Group 2, n=70). Thoracic aortic calcium score was determined using a 64-slice computed tomography system (Siemens, Germany).</p></sec><sec><title>Results</title><p>Results. No significant differences were found between the groups in the key clinical and anthropometric characteristics. The Agatston calcium score ranged from 312 to 2738 and was higher in the group of uncontrolled HTN (p&lt;0,00001). ROC analysis found that calcium score of 900 with a sensitivity of 71,4% and a specificity of 66,2% differentiates individuals with controlled and uncontrolled resistant HTN (AUC=0,791). According to ABPM, the most significant differences were obtained when comparing pulse pressure, 24-hour systolic blood pressure (BP) index, morning surge, and daytime systolic BP variability. Regardless of the effectiveness of antihypertensive therapy, the calcium score correlated positively with the variability of systolic BP, pulse BP, and morning surge, and negatively with the 24-hour systolic BP index, but the relationship strength was greater when the target BP was not reached.</p></sec><sec><title>Conclusion</title><p>Conclusion. In patients with resistant HTN without BP control, higher values of pulse pressure, morning surge, and systolic BP variability, and a decrease in the 24-hour systolic BP index were noted.
