Изучить влияние 48-недельной терапии аторвастатином и эзетимибом на лабораторные показатели, структурно функциональные характеристики артерий и маркеры хронической сердечной недостаточности в постинфарк­тном периоде.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Включено 87 пациентов с острым инфарктом миокарда, которым в первые 24 ч назначали аторвастатин 80 мг/сут. Во время госпитализации, через 5-6, 24, 48 нед. проводили клинико лабораторное и инструментальное обследование. При уровне холестерина липопротеидов низкой плотности (ХС ЛНП) &gt;1,4 ммоль/л и снижении &lt;50% на одном из повторных визитов дополнительно назначали эзетимиб 10 мг/сут.</p></sec><sec><title>Результаты</title><p>Результаты. Исследование завершил 81 человек (93%). Больных разделили на группы: 1-я (n=32) — монотерапия аторвастатином; 2-я (n=49) — пациенты дополнительно получали эзетимиб. В 1 группе ХС ЛНП снизился через 48 нед. на 53% (р&lt;0,001), во 2 группе на 63,2% (р&lt;0,001). По результатам ультразвукового исследования общих сонных артерий технологией RF во 2 группе выявлено снижение толщины комплекса интима медиа через 24 и 48 нед. на 9,1% (р&lt;0,001) и 10,5% (р&lt;0,001) по сравнению с исходным значением, в 1 группе — на 4,5% только на 24 нед. (р=0,012). При анализе эндотелиальной функции произошло увеличение потокозависимой вазодилатации только во 2 группе с 9,1 (5,6; 11,8)% до 14,3±6,8% через 48 нед. (р&lt;0,001). При добавлении эзетимиба отмечен регресс N-терминального фрагмента предшественника мозгового натрийуретического пептида через 24 нед. на 69,6% (р=0,005), через 48 нед. — на 72,4% (р=0,009). Во 2 группе показатель снизился к окончанию наблюдения на 75,5% (р=0,010).</p></sec><sec><title>Заключение</title><p>Заключение. Результаты подтверждают целесообразность добавления эзетимиба к статинам у пациентов очень высокого риска в связи с максимально выраженным гиполипидемическим действием, улучшением структурно функциональных свой­ств общих сонных артерий, эндотелиальной функции и клинико-лабораторных маркеров хронической сердечной недостаточности.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the effect of 48-week therapy with atorvastatin and ezetimibe on laboratory parameters, structural and functional arterial characteristics and heart failure markers in the post-infarction period.</p></sec><sec><title>Material and methods</title><p>Material and methods. A total of 87 patients with acute myocardial infarction were included. In the first 24 hours, atorvastatin 80 mg/day was prescribed. During hospitalization, after 5-6, 24, 48 weeks, clinical and paraclinical examinations were performed. In the level of low-density lipoprotein cholesterol (LDL-C) &gt;1,4 mmol/l and the decrease &lt;50% at one of the follow-up visits, ezetimibe 10 mg/day was additionally prescribed.</p></sec><sec><title>Results</title><p>Results. Eighty participants (93%) completed the study. Patients were divided into following groups: group 1 (n=32) — atorvastatin monotherapy; group 2 (n=49) — ezetimibe and atorvastatin therapy. In group 1, LDL-C decreased after 48 weeks by 53% (p&lt;0,001), while in group 2 by 63,2% (p&lt;0,001). According to carotid ultrasound in group 2, a decrease in the intima media thickness after 24 and 48 weeks was revealed by 9,1% (p&lt;0,001) and 10,5% (p&lt;0,001) compared to the baseline value, while in group 1 — by 4,5% only on the 24th week (p=0,012). When analyzing endothelial function, there was an increase in flow-dependent vasodilation only in group 2 from 9,1 (5,6; 11,8)% to 14,3±6,8% after 48 weeks (p&lt;0,001). With the addition of ezetimibe, there was a regression of the N-terminal pro-brain natriuretic peptide after 24 weeks by 69,6% (p=0,005), after 48 weeks — by 72,4% (p=0,009). In group 2, it decreased by 75,5% by the end of follow-up (p=0,010).</p></sec><sec><title>Conclusion</title><p>Conclusion.
