Установить предикторы расширения полости левого желудочка (ЛЖ) и формирования постинфарктной аневризмы ЛЖ (ПАЛЖ) у пациентов с инфарктом миокарда с подъемом сегмента ST электрокардиограммы (ИМпST) и/или формированием зубца Q электрокардиограммы (ЭКГ).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведено регистровое исследование пациентов, поступивших с ИМпST и/или формированием зубца Q ЭКГ в первые 24 ч от начала заболевания в период с 1 ноября 2022г по 31 марта 2023г. В исследование включено 138 пациентов. Средний возраст пациентов составил 62±11 года. Лечение и обследование проводилось согласно действующим клиническим рекомендациям Министерства здравоохранения Российской Федерации (2020г) "Острый инфаркт миокарда с подъемом сегмента ST электрокардиограммы". У пациентов были определены уровни стимулирующего фактора роста (sST2), пропротеин конвертазы субтилизин-кексин 9 типа, N-концевого промозгового натрийуретического пептида и высокочувствительного С-реактивного белка методом иммуноферментного анализа в первые сутки заболевания. Пациенты были разделены на две группы: 1 группа — пациенты с расширением ЛЖ/формированием ПАЛЖ, n=25 (18,1%), 2 группа — пациенты без нарушения геометрии ЛЖ, n=113 (81,9%). Для определения независимых предикторов расширения ЛЖ/формирования ПАЛЖ выполняли однофакторный и многофакторный регрессионный анализ.</p></sec><sec><title>Результаты</title><p>Результаты. Данное исследование показало, что при возрастании уровня sST2 на 1 нг/л увеличиваются шансы расширения ЛЖ/формирования ПАЛЖ в 1,53 раза. Передняя локализация ИМ повышает шансы расширения ЛЖ/ формирования ПАЛЖ в 63,55 раза. Увеличение рСКФ на 2 сутки госпитализации на 1 мл/мин/1,73 м2 уменьшает шансы расширения ЛЖ/формирования ПАЛЖ в 1,07 раза.</p></sec><sec><title>Заключение</title><p>Заключение. Исследование показало, что передняя локализация инфаркта миокарда и увеличение уровня sST2 повышают шансы расширения ЛЖ/формирования ПАЛЖ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To establish predictors of left ventricular (LV) dilatation and post-infarction left ventricular aneurysm (LVA) development in patients with ST-segment elevation myocardial infarction (STEMI) and/or with Q wave.</p></sec><sec><title>Material and methods</title><p>Material and methods. This registry study included patients admitted with STEMI and/or with Q wave in the first 24 hours from the disease onset in the period from November 1, 2022 to March 31, 2023. The study included 138 patients. The mean age of the patients was 62±11 years. Treatment and examination were carried out in accordance with the current Russian clinical guidelines (2020) on STEMI. The levels of stimulating growth factor expressed by genome 2 (sST2), proprotein convertase subtilisin-kexin type 9, N-terminal pro-brain natriuretic peptide and high-sensitivity C-reactive protein were determined in patients by enzyme immunoassay on the first day of the disease. The patients were divided into two following groups: group 1 — patients with LV dilatation/LVA, n=25 (18,1%), group 2 — patients without LV geometry disorders, n=113 (81,9%). Univariate and multivariate regression analyzes were performed to determine independent predictors of LV dilation/postinfarction LVA.</p></sec><sec><title>Results</title><p>Results. This study showed that with an increase in sST2 levels by 1 ng/L, the probability of LV dilation/LVA formation increase by 1,53 times. Anterior location of myocardial infarction increases the probability of LV dilation/LVA formation by 63,55 times. An increase in eGFR on day 2 of hospitalization by 1 ml/min/1,73 m2 reduces the probability of LV dilatation/LVA formation by 1,07 times.</p></sec><sec><title>Conclusion</title><p>Conclusion.
