Оценка динамики артериальной жесткости, центрального артериального давления (ЦАД) и определение потенциально влияющих на них факторов у пациентов с аневризмой нисходящего грудного и брюшного отделов аорты, в сочетании с артериальной гипертензией, через год после эндоваскулярной изоляции аневризмы аорты ((T)EVAR).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Включались пациенты ≥18 лет без клинически значимых нарушений ритма сердца. До операции и через год выполнялось обследование: определение периферического артериального давления, частоты сердечных сокращений (ЧСС), ЦАД, индекса аугментации (AIx), давления в точке максимума подъема прямой пульсовой волны (PT1h), времени возврата отраженной волны (Тr) и каротидно-феморальной скорости распространения пульсовой волны (кфСРПВ).</p></sec><sec><title>Результаты</title><p>Результаты. Включено 50 пациентов (средний возраст 68±9 лет, 41 мужчина, 16 — с аневризмой нисходящего грудного отдела аорты, 34 — с аневризмой брюшного отдела аорты), обследовано повторно 23 пациента. После (T) EVAR выявлено увеличение кфСРПВ (+2,6 м/с, р&lt;0,001), PT1h (+5 мм рт.ст., р=0,026), периферического пульсового давления (+7 мм рт.ст., р=0,039), ЧСС (+5 уд./мин, р=0,025). Показатели ЦАД и AIx не изменились. Найдена зависимость снижения Tr от длины стент-графта (β=0,42) и снижения ЧСС (β=-0,52). За год зарегистрировано 6 сердечно-сосудистых осложнений.</p></sec><sec><title>Заключение</title><p>Заключение. Имплантация эндопротеза сопровождается повышением кфСРПВ вне зависимости от локализации аневризмы. Повышение PT1h через год после (T)EVAR может отражать снижение растяжимости аорты, ухудшение демпфирующей функции. Отсутствие динамики ЦАД, AIх, вероятно, объясняется особенностями отраженной волны после операции. Предикторы увеличения Tr — снижение ЧСС и имплантация более протяженных стент-графтов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To assess the changes of arterial stiffness, central blood pressure (CBP), and determine the factors potentially influencing them in patients with descending thoracic and abdominal aortic aneurysm, in combination with hypertension, one year after endovascular aneurysm repair ((T)EVAR).</p></sec><sec><title>Material and methods</title><p>Material and methods. Patients ≥18 years of age without clinically significant cardiac arrhythmias were included. Before surgery and a year later, we determined peripheral blood pressure, heart rate (HR), CBP, augmentation index (AIx), pressure at the point of maximum rise of the direct pulse wave (PT1h), time to return of the reflected pressure wave (Tr) and carotid femoral pulse wave velocity (cfPWV).</p></sec><sec><title>Results</title><p>Results. Fifty patients were included (mean age, 68±9 years; 41 men; 16 with descending thoracic aortic aneurysm, 34 with abdominal aortic aneurysm), 23 patients were reexamined. After (T)EVAR, an increase in cfPWV (+2,6 m/s, p&lt;0,001), PT1h (+5 mm Hg, p=0,026), peripheral pulse pressure (+7 mm Hg, p=0,039), heart rate (+5 beats/min, p=0,025). The CBP and AIx values have not changed. The dependence of Tr decrease on the stent-graft length (β=0,42) and the decrease in heart rate (β=-0,52) was found. During the year, 6 cardiovascular events were registered.</p></sec><sec><title>Conclusion</title><p>Conclusion. Graft implantation is accompanied by an increase in cfPWV, regardless of aneurysm location. An increase in PT1h one year after (T)EVAR may reflect a decrease in aortic distensibility and worsening damping function. No changes of CBP and AIx is probably explained by reflected wave characteristics after surgery.
