Изучить эффективность и безопасность применения ацетазоламида, назначаемого внутрь пациентам с декомпенсированной хронической сердечной недостаточностью (ХСН) на госпитальном этапе в течение 72 ч с момента поступления, по сравнению со стандартной терапией.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В открытое проспективное рандомизированное многоцентровое исследование планируется включить 400 пациентов, поступивших в экстренном порядке с декомпенсацией ХСН II-IV функционального класса NYHA, находящихся на стационарном лечении: по 200 пациентов в группе стандартной терапии и группе с добавлением ацетазоламида в таблетированной форме. Первичная конечная точка включает количество пациентов, достигших компенсации в соответствии с критериями прекращения диуретической терапии. Вторичные конечные точки следующие: увеличение объема выделенной мочи в первые 72 ч госпитализации (с момента рандомизации), снижение веса, натрийурез (оценка в суточной моче), длительность госпитализации, длительность пребывания в отделении реанимации и интенсивной терапии, смерть от любых причин в течение 90 дней, смерть от сердечно-сосудистых заболеваний в течение 90 дней, смерть от декомпенсации ХСН или острой декомпенсации сердечной недостаточности в течение 90 дней, количество пункций плевры и перикарда, выполненных за период госпитализации, количество баллов по шкале ШОКС при выписке из стационара, тест 6-минутной ходьбы при выписке из стационара. Планируемая длительность наблюдения — госпитальный период с оценкой клинической картины и лабораторных параметров. Телемедицинский контакт с пациентами проводится через 14, 30 и 90 дней с использованием структурированного опросника.</p></sec><sec><title>Заключение</title><p>Заключение. Анализ клинических, лабораторных и эхокардиографических параметров на фоне применения таблетированной формы ацетазоламида позволит определить критерии эффективности диуретической терапии у пациентов с декомпенсацией ХСН в краткосрочном и долгосрочном периодах.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the efficacy and safety of acetazolamide administered orally to patients with decompensated heart failure (HF) at the hospital stage within 72 hours from admission, compared with standard therapy.</p></sec><sec><title>Material and methods</title><p>Material and methods. This open-label, prospective, randomized, multicenter study is planned to include 400 patients urgently admitted to hospital with NYHA class II-IV decompensated HF: 200 patients each in the standard therapy group and additional acetazolamide (tablets) group. The primary endpoint includes the number of patients who achieved compensation in accordance with the criteria for diuretic therapy discontinuation. There are secondary endpoints: increase in urine output in the first 72 hours of hospitalization (since randomization), weight loss, 24-hour natriuresis, length of stay in hospital, length of stay in the intensive care unit, 90-day any-cause death, 90-day cardiovascular death, 90-day death due to chronic HF decompensation or acute decompensated HF, the number of pleuro- and pericardiocentesis episodes during the hospitalization, scale for clinical condition assessment of HF patient (SHOKS) at discharge from the hospital, 6-minute walk test at discharge from the hospital. The planned follow-up duration is a hospital period with an assessment of the clinical picture and laboratory parameters. Telemedicine contact with patients is carried out after 14, 30 and 90 days using a structured questionnaire.</p></sec><sec><title>Conclusion</title><p>Conclusion.
