Создание модели дифференцированного выбора стратегии реваскуляризации у больных инфарктом миокарда с подъемом сегмента ST (ИМпST) и многососудистым поражением (МП) коронарного русла.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование было включено 327 пациентов с ИМпST и МП коронарного русла, подвергающихся первичному чрескожному коронарному вмешательству (ЧКВ). Больные были распределены в две группы: (1) Многососудистое стентирование (МС) в рамках первичного ЧКВ (n=91); (2) Поэтапная реваскуляризация (ПР) (n=236). Конечными точками исследования на протяжении 12 месяцев наблюдения были значимые неблагоприятные кардиоваскулярные события. Внутри каждой группы была оценена относительная частота наступления неблагоприятного исхода и рассчитан прогностический коэффициент, позволяющий определить значимость каждого учитываемого клинико-демографического и ангиографического показателя в развитии неблагоприятного исхода.</p></sec><sec><title>Результаты</title><p>Результаты. Наибольшей негативной прогностической значимостью для неблагоприятного исхода у пациентов, подвергающихся стратегии МС в рамках первичного ЧКВ, обладали следующие факторы: (1) наличие постинфарктного кардиосклероза (ПИКС), (2) тяжелый коронарный атеросклероз (SYNTAX ≥23 балла), (4) пожилой возраст (≥65 лет), (5) женский пол. Тогда как в отношении ПР: (1) наличие в анамнезе инсульта, (2) ПИКС, (3) мультифокальный атеросклероз, (4) трехсосудистое поражение коронарного русла и (5) применение стентов без лекарственного покрытия.</p></sec><sec><title>Заключение</title><p>Заключение. Учет комплекса клинико-демографических и анатомо-ангиографических факторов может способствовать улучшению результатов лечения при ИМпST посредством определения оптимальной стратегии реваскуляризации.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. Invention of the model of differentiated strategy selection of revascularization in acute myocardial infarction with ST elevation (STEMI) and multivessel disease (MVD).</p></sec><sec><title>Material and methods</title><p>Material and methods. Totally, 327 patients with STEMI and MVD included, undergoing primary transcutaneous coronary intervention (PCI). Patients were selected into 2 groups: (1) Multivessel stenting (MVS) during primary PCI (n=91); (2) Staged revascularization (SR) (n=236). The endpoints for 12 months follow-up were significant adverse cardiovascular events. Inside every group the relative level of an adverse outcome was calculated and prognostic coefficient invented that makes possible to estimate significance of every including clinical and demographic and angiographic parameter in the development of adverse outcome.</p></sec><sec><title>Results</title><p>Results. The most negative prognostic significance for adverse outcome in patients undergoing MVS for primary PCI had the following factors: (1) post infarction cardiosclerosis, (2) severe coronary atherosclerosis (SYNTAX ≥23 points), (3) older age (≥65 y.o.), (4) female gender. Concerning SR: (1) anamnesis of stroke, (2) post infarction cardiosclerosis, (3) multifocal atherosclerosis, (4) three-vessel coronary disease (5) non-drug eluting stents.</p></sec><sec><title>Conclusion</title><p>Conclusion.
