Определить возможность интегральной оценки состояния сердечно-сосудистой системы (ССС) при артериальной гипертензии (АГ) на основе параметров микроциркуляции, используя методы неинвазивной цифровой капилляроскопии и макроциркуляции. Материал и методы. В исследование включено 188 человек от 40 лет и старше:32 пациента с А Д 120–139 мм рт.ст., 36 пациентов — с А Г 1–2 степени, 85 пациентов с А Г, у которых на фоне гипотензивной терапии достигнут целевой уровень АД.Контрольную группу составили 35 здоровых добровольцев без признаков патологии ССС. Всем испытуемым проводилось клинико-лабораторное обследование: офисное измерение АД, суточное мониторирование АД, ЭхоКГ, исследованиемикроциркуляции ногтевого ложа пальца, определение скорости распространенияпульсовой волны и функции эндотелия, также измерение АД и пульса.Результаты. Выявлено увеличение коэффициента ремоделирования (Кво/ао)в группах “Пред АГ”, “АГ” и “АГ леч” по сравнению с группой “Здоровые”. СРПВ быладостоверно выше в группах “Пред АГ” и “АГ” по сравнению с группой “Здоровые”.На основе параметров макро- и микроциркуляции вычислялся интегральныйиндекс состояния ССС, значения которого были достоверно выше в группе “АГ”и существенно снижались на фоне эффективной терапии.Заключение. В результате проведенных исследований на основе параметровмакро- и микроциркуляции разработан интегральный индекс оценки состоянияССС у пациентов с А Г.Индекс достаточно хорошо отражает динамику изменений макро- и микроциркуля-ции при АГ на фоне приема антигипертензивной терапии, что позволяет использо-вать его как индикатор эффективности лечения.Использование комплексного определения параметров макро- и микроциркуля-ции позволяет выявить наличие признаков АГ уже в раннем периоде заболевания.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To assess the potential of the integral assessment of cardiovascular system status in patients with arterial hypertension (AH), based on the evaluation of the parameters of microcirculation (non-invasive digital capillaroscopy) and  macrocirculation (pulse wave velocity (PWV) and endothelial function).Material and methods. The study included 188 people aged 40 years and older:32 patients with systolic blood pressure (BP) levels 120–139 mm Hg (“PreAH”group), 36 patients with Stage 1–2 AH (“AH” group), and 85 AH patients whoachieved target BP levels due to antihypertensive therapy (“Treated AH” group). Thecontrol group included 35 healthy volunteers, without any signs or symptoms ofcardiovascular disease (“Healthy” group). All participants underwent clinical andlaboratory examination, including office BP measurement, 24-hour BP monitoring,and echocardiography, the assessment of nailfold microcirculation, pulse wavevelocity, endothelial function, as well as the measurement of BP and heart rate.Results. There was an increase in remodelling coefficient (Cv/a) in the groups“PreAH”, “AH”, and “Treated AH”, compared to the “Healthy” group. PWV levels weresignificantly higher in “PreAH” and “Treated AH” groups than in the “Healthy” group.Based on micro- and macrocirculation parameters assessed, the integral index ofcardiovascular status was calculated. The values of this index were significantly higher in the “AH” group. Effective antihypertensive treatment was associated with areduction in these values.Conclusion.
