Исследовать взаимосвязь между рентгенологическими характеристиками эпикардиальной жировой ткани (ЭЖТ) и симпатической активностью миокарда, а также изучить их ассоциацию с развитием поздних рецидивов фибрилляции предсердий (ФП) после радиочастотной аблации (РЧА).</p></sec><sec><title>Материал и  методы</title><p>Материал и  методы. В исследование проспективно было включено 26 человек с персистирующей и длительно персистирующей ФП, которым планировалось интервенционное лечение ФП. Перед процедурой РЧА всем пациентам проводили сцинтиграфию миокарда с 123I-метайодбензилгуанидином (123I-МИБГ) для оценки симпатической иннервации миокарда и мультиспиральную компьютерную томографию сердца с внутривенным болюсным контрастированием для оценки анатомии легочных вен, объема левого предсердия, объема ЭЖТ. Клиническое наблюдение, включая мониторинг ритма с использованием электрокардиографии (ЭКГ) в 12 отведениях и суточное мониторирование ЭКГ, осуществлялось через 3, 6 и 12 мес. после РЧА.</p></sec><sec><title>Результаты</title><p>Результаты. После окончания наблюдения пациенты были разделены на две группы: с рецидивом ФП (группа 1, n=8) и без рецидива ФП (группа 2, n=18). По данным многофакторного логистического анализа установлено, что только показатель глобальной симпатической активности — скорость вымывания 123I-МИБГ (отношение шансов 1,0943; 95% доверительный интервал 1,0138- 1,1812) проявил себя как независимый предиктор развития поздних рецидивов ФП после РЧА. На основании ROC-анализа было установлено, что значение скорости вымывания 123I-МИБГ &gt;21% с чувствительностью 75% и специфичностью 83,3% (AUC=0,844; р&lt;0,001) позволяет прогнозировать развитие поздних рецидивов ФП после РЧА.</p></sec><sec><title>Заключение</title><p>Заключение. Параметры глобальной симпатической активности сердца, оцененные посредством сцинтиграфии миокарда с 123I-МИБГ, связаны с развитием поздних рецидивов ФП после РЧА у пациентов с персистирующей и длительно персистирующей ФП. Достоверных данных, доказывающих наличие ассоциаций между параметрами симпатической иннервации миокарда и рентгенологическими показателями ЭЖТ, а также влияние последних на риск развития рецидивов ФП после процедуры РЧА, получено не было.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To investigate the relationship between radiological characteristics of epicardial adipose tissue (EAT) and myocardial sympathetic activity, as well as to study their association with late recurrence of atrial fibrillation (AF) after radiofrequency ablation (RFA).</p></sec><sec><title>Material and methods</title><p>Material and methods. This prospective study included 26 people with persistent and long-standing persistent AF scheduled for interventional AF treatment. Before the RFA procedure, all patients underwent cardiac 123I-metaiodobenzylguanidine (123I-MIBG) scintigraphy to assess the myocardial sympathetic innervation and contrast-enhanced cardiac multislice computed tomography to assess pulmonary vein anatomy, left atrial volume, and EAT volume. Clinical follow-up, including 12-lead electrocardiography (ECG) and 24-hour ECG monitoring, was carried out 3, 6 and 12 months after RFA.</p></sec><sec><title>Results</title><p>Results. After the end of follow-up, the patients were divided into two groups: with AF recurrence (group 1, n=8) and without AF recurrence (group 2, n=18). Multivariate logistic analysis found that only the 123I-MIBG washout rate (odds ratio, 1,0943; 95% confidence interval, 1,0138-1,1812) proved to be an independent predictor of late AF recurrence after RFA. ROC analysis revealed that a 123I-MIBG washout rate &gt;21% with a sensitivity of 75% and a specificity of 83,3% (AUC=0,844; p&lt;0,001) predicts late AF recurrence after RFA.</p></sec><sec><title>Conclusion</title><p>Conclusion. Parameters of myocardial sympathetic activity, assessed by 123I-MIBG myocardial scintigraphy, are associated with late AF recurrence after RFA in patients with persistent and long-standing persistent AF.
