Изучение структурных и функциональных параметров левых отделов сердца у пациентов с выраженным аортальным стенозом (АС) и сохраненной фракцией выброса (ФВ) с целью определения риска развития фибрилляции предсердий (ФП).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включены 84 пациента (37 мужчин), средний возраст 68±8 лет, с выраженным АС и ФВ &gt;55%. Все пациенты имели синусовый ритм и были бессимптомными. Эхокардиография проводилась с определением продольной деформации (ПД) левого желудочка (ЛЖ), правого желудочка, левого предсердия (ЛП) и жесткости левого предсердия (ЖЛП) методом speckle tracking. Также определялись индекс массы (ИМ) ЛЖ, и индекс максимального объема левого предсердия (ИОЛП). Пациенты наблюдались в течение 1 года.</p></sec><sec><title>Результаты</title><p>Результаты. ФП была зарегистрирована у 27 (32%) пациентов, из коих 9 (33%) имели бессимптомные эпизоды ФП, выявленные 48 ч электрокардиографическим наблюдением. 18 (67%) пациентов с ФП чувствовали сердцебиение. Пациенты с эпизодами ФП и без нее имели недостоверные различия в ИМ ЛЖ, ИОЛП, ПД ЛЖ. Пациенты с ФП имели более низкую ПД ЛП и более высокую ЖЛП по сравнению с пациентами без развития ФП. Регрессионный анализ всех параметров выявил, что ПД ЛП и ЖЛП являлись независимыми предикторами развития ФП.</p></sec><sec><title>Заключение</title><p>Заключение. ФП развивается у примерно одной трети бессимптомных пациентов с выраженным АС и нормальной ФВ. Развитие ФП предрасполагает к появлению симптомов АС у большинства пациентов. ПД ЛП и ЖЛП являлись предикторами ФП у этих пациентов. Выявление пациентов с риском развития ФП позволит провести замену аортального клапана в более ранние сроки.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the structural and functional left heart parameters in patients with severe aortic stenosis (AS) and preserved ejection fraction (EF) in order to determine the risk of atrial fibrillation (AF).</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 84 patients (men, 37; mean age, 68±8 years) with severe AS and EF &gt;55%. All patients had sinus rhythm and were asymptomatic. Echocardiography was performed to assess longitudinal strain of the left ventricle (LVLS), right ventricle, left atrium (LALS) and the left atrial stiffness (LAS) using the speckle tracking method. Left ventricular mass index (LVMI) and maximum left atrium volume index (LAVI) were also determined. Patients were followed up for 1 year.</p></sec><sec><title>Results</title><p>Results. AF was reported in 27 (32%) patients, of which 9 (33%) had asymptomatic AF episodes detected by 48-hour electrocardiography. Eighteen (67%) patients with AF felt palpitations. Patients with and without episodes of atrial fibrillation had non-significant differences in LVMI, LAVI, and LVLS. Patients with atrial fibrillation had a lower LALS and a higher LAS compared with patients without atrial fibrillation. Regression analysis revealed that LALS and LAS were independent predictors of AF.</p></sec><sec><title>Conclusion</title><p>Conclusion. AF develops in about one third of asymptomatic patients with severe AS and normal EF. The development of AF predisposes to the onset of AS symptoms in most patients. LALS and LAS were predictors of AF in these patients.
