В статье описаны ключевые компоненты медицинской помощи при сердечной недостаточности (преемственность, маршрутизация пациентов, образовательные мероприятия) и приведены примеры документов, протоколов, чек-листов, которые могут быть использованы в реальной клинической практике специалистами медицинских организациях в регионах Российской Федерации.</p></abstract><trans-abstract xml:lang="en"><p>The extent of the problem of heart failure determines the priority of the intention of developing and implementing an optimal model of medical care for this group of patients.
