Цель. Изучить влияние поведенческого типа Д на психологический статус (ПС) и качество жизни у пациентов с ишемической болезнью сердца через год после коронарного шунтирования.

Материал и методы

Материал и методы. Оценка ПС до и после проведения КШ выполнена у 408 больных, из которых было сформировано 2 группы: пациенты с наличием типа Д (n=93), и 2 группа — пациенты без типа Д (n=315). Изучение ПС проводилась с помощью опросников Спилбергера-Ханина, шкалы депрессии, SF-36, DS-14. Результаты. Через год после операции коронарного шунтирования отмечено уменьшение числа лиц с высоким уровнем тревожности, улучшение качества жизни по шкалам физического функционирования и физического состояния (р<0,05). Распространенность типа личности Д в динамике не изменилась, что свидетельствует о стабильности появлений этого хронического психологиче­ского фактора риска. Отмечено также неблагоприятное влияние факта нали­чия типа личности Д на уровень депрессии, тревожности и качества жизни через год после операции коронарного шунтирования. При множественном логистическом регрессионном анализе независимым предиктором низкого уровня как физического, так и психологического компонентов КЖ было нали­чие типа личности Д, а также повышенный уровень личностной тревожности и депрессии.

Заключение

Заключение. Склонность к психологическому дистрессу является стабиль­ной личностной характеристикой больных до и через год после КШ, сопрово­ждается снижением как психологического, так и физического компонента КЖ Оценка личностных особенностей пациентов перед КШ может помочь в прове­дении целенаправленных мероприятий, направленных на повышение их КЖ.

Aim

Aim. To investigate the effects of Type D personality traits on psychological status (PS) and quality of life (QoL) in patients with coronary heart disease one year after coronary artery bypass graft surgery (CABG).

Material and methods

Material and methods. PS was assessed at baseline and after CABG in 408 patients, who were divided into two groups: with Type D personality traits (n=93; Group 1) and without these traits (n=315; Group 2). The Spielberger-Khanin, depression, SF-36, and DS-14 scales were used for PS assessment. Results. One year after CABG, fewer patients reported high levels of anxiety, and there was an improvement in physical functioning and physical status QoL domains (p<0,05). The percentage of patients with Type D personality traits did not change over time, suggesting that this chronic psychological risk factor is relatively stable. Type D personality traits negatively affected the levels of anxiety, depression, and QoL one year after CABG. According to the results of multivariate logistic regression analyses, type D personality and elevated levels of trait anxiety of psychological and physical QoL parameters.

Conclusion

Conclusion. The tendency towards psychological distress, a stable personality trait observed both before and one year after CABG, was associated with reduced psychological and physical QoL parameters. The pre-CABG assessment of personality traits could optimise the complex management strategy and improve QoL.
