Оценить влияние геморрагического пропитывания миокарда (ГПМ) на структурно-функциональные показатели миокарда по данным эхокардиографии (ЭхоКГ) у пациентов с первичным инфарктом миокарда с подъемом сегмента ST (ИMnST).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование последовательно включено 60 пациентов с первичным ИМпST. На вторые сутки после острого коронарного события всем исследуемым проводилась магнитно-резонансная томография сердца с парамагнитным контрастным усилением. ГПМ визуализировалось как гипо-интенсивные участки на фоне миокарда с повышенной интенсивностью сигнала в Т2-взвешенном режиме. Далее всем пациентам выполнялся стандартный протокол ЭхоКГ на седьмые сутки после инфаркта миокарда.</p></sec><sec><title>Результаты</title><p>Результаты. Феномен ГПМ встречался у 31 пациента, в 51,6% случаев. При этом наиболее часто, у 22 пациентов (70,9%), ГПМ выявлялось в сочетании с микроваскулярной обструкцией (МВО). У 9 пациентов (29%) из 31 визуализировался изолированный феномен ГПМ. Более низкие значения фракции выброса (ФВ) левого желудочка (ЛЖ) и объемных показателей ЛЖ ассоциировались с сочетанием МВО и ГПМ. В то же время показатели объемных характеристик и ФВ ЛЖ при изолированном ГПМ были сопоставимы с группой, где феномены микрососудистого повреждения отсутствовали. При количественной оценке ГПМ к площади ЛЖ продемонстрировано, что ГПМ занимает 1% (1-3%). Проведение корреляционного анализа показало умеренную обратную корреляцию между площадью ГПМ и сократительной функцией ЛЖ: чем больше площадь ГПМ, тем ниже ФВ ЛЖ (R=-0,35; p=0,007).</p></sec><sec><title>Заключение</title><p>Заключение. Проведен анализ влияния разных фенотипов ГПМ на структурно-функциональные показатели миокарда по данным ЭхоКГ в ранний постинфарктный период у пациентов с первичным ИМпST. Показано, что комбинация ГПМ с МВО и изолированное ГПМ по-разному влияют на сократительную функцию ЛЖ. Сочетание ГПМ с МВО является предиктором снижения ФВ ЛЖ и расширения конечно-систолического объема, конечно-систолического индекса, в то время как изолированное ГПМ не влияет на изменение данных показателей. Продемонстрированы корреляции между площадью ГПМ и снижением ФВ ЛЖ, а также увеличением конечно-систолического индекса.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To analyze the effect of intramyocardial haemorrhage (IMH) on the structural and functional echocardiographic parameters of myocardium in patients with primary ST-segment elevation myocardial infarction (STEMI).</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 60 patients with primary STEMI reperfused within 12 hours after symptom onset. On the second day after the event, all subjects underwent gadolinium-enhanced cardiac magnetic resonance imaging (MRI). IMH was visualized as T2-weighted hypointense areas. Subsequently, all patients underwent the standard echocardiography on the 7th day after MI.</p></sec><sec><title>Results</title><p>Results. IMH was revealed in 31 patients (51,6%). In 22 patients (70,9%), IMH was accompanied by microvascular obstruction (MVO). In the remaining 9 patients (29%), an isolated IMH phenomenon was visualized. Lower values of left ventricular ejection fraction (LVEF) and LV volume parameters were associated with a combination of MVO and IMH. At the same time, the indices of volumetric characteristics and LVEF in isolated IMH were the same as in the group without IMH and MVO. It was demonstrated that the IMH occupies 1% (1-3%) of the LV myocardium. Correlation analysis showed a moderate inverse correlation between the IMH area and LV contractile function: the larger the area, the lower the LVEF (R=-0,35; p=0,007).</p></sec><sec><title>Conclusions</title><p>Conclusions. The analysis of the influence of different IMH phenotypes on the structural and functional echocardiographic parameters of myocardium in the short-term period after STEMI has shown that the combination of IMH with MVO and isolated IMH have different effects on LV contractile function. The combination of IMH with MVO is a predictor of a decrease in LVEF and increase of end-systolic volume (ESV), while an isolated IMH does not affect these parameters.
