Определить независимые лабораторно-инструментальные предикторы повторного инфаркта миокарда (ИМ) у больных трудоспособного возраста и на основании полученных результатов предложить модель прогнозирования повторного коронарного события у данной категории больных.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследовании участвовали 424 пациента (медиана возраста 50 (43,5; 55,0) лет). Все пациенты в 2017г проходили лечение в региональном сосудистом центре г. Перми по поводу впервые возникшего ИМ. Ретроспективно изучали данные историй болезни (анамнез, сведения о сопутствующих диагнозах, лабораторные и инструментальные данные, лечение на стационарном этапе). Через 2 года сведения о развитии повторного ИМ собирали путём анализа данных электронной медицинской документации пациентов в Региональной информационноаналитической системе здравоохранения Пермского края. В зависимости от исхода заболевания все больные были разделены на две группы: с повторным ИМ (n=78) и без него (n=346). С помощью пакета прикладных программ SPSS v.20, v.23 проводили сравнение основных демографических, клинико-лабораторных и инструментальных параметров в группах, полученных исходно при госпитализации. Для определения независимых предикторов повторного ИМ выполняли однофакторный и многофакторный регрессионный анализ.</p></sec><sec><title>Результаты</title><p>Результаты. В ходе проведения многофакторного регрессионного анализа установлены следующие независимые предикторы развития повторного ИМ: фракция выброса (ФВ) левого желудочка (ЛЖ) &lt;50% (отношение шансов (ОШ)=5,5; 95% доверительный интервал (ДИ): 1,56-19,34; p=0,008), анемия при поступлении (ОШ=2,95; 95% ДИ: 1,089-9,765; p=0,046), многососудистое поражение коронарных артерий (КА) (ОШ=2,24; 95% ДИ: 1,285-3,909; p=0,004). На примере больных, перенесших ИМ в молодом и среднем возрасте, получена логистическая регрессионная модель, позволяющая с чувствительностью 73,7% и специфичностью 79,6% прогнозировать вероятность развития повторного ИМ в течение 2 лет с момента первичной госпитализации.</p></sec><sec><title>Заключение</title><p>Заключение. Результаты настоящего исследования свидетельствуют о том, что при стратификации риска развития повторного ИМ у пациентов трудоспособного возраста, возможно, следует учитывать снижение ФВ ЛЖ, уровня гемоглобина и многососудистое поражение КА.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To determine independent predictors of recurrent myocardial infarction (MI) and to create a model for predicting recurrent coronary events in working-age patients.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 424 patients (median age 50 (43,5; 55,0) years). In 2017, all patients underwent treatment at the Perm Regional Vascular Center due to the first MI. We retrospectively analyzed the patient data with regard to medical history, comorbidities, diagnostic results, and treatment. After 2 years, information on recurrent MI was collected by analyzing data from electronic medical records of patients. Depending on the outcome, all patients were divided into two groups: with (n=78) and without (n=346) recurrent MI. Using the SPSS Statistics v.20, v.23 software package, we compared the central demographic, clinical, diagnostic parameters in the groups. Univariate and multivariate regression analyzes were performed to determine independent predictors of recurrent MI.</p></sec><sec><title>Results</title><p>Results. Multivariate regression established the following independent predictors of recurrent MI: left ventricular ejection fraction &lt;50% (odds ratio (OR) 5,5, 95% confidence interval (CI) 1,56-19,34, p=0,008), anemia (OR=2,95, 95% CI 1,089- 9,765, p=0,046), multivessel coronary artery disease (OR 2,24, 95% CI 1,285- 3,909, p=0,004). Logistic regression model was created that allows predicting the 2-year risk of recurrent MI after the initial hospitalization with a sensitivity of 73,7% and a specificity of 79,6%.</p></sec><sec><title>Conclusion</title><p>Conclusion.
