Изучить механизмы формирования кальций-фосфатных бионов (КФБ), являющихся одним из триггеров дисфункции эндотелия, в крови пациентов с коронарным (КоА) и каротидным атеросклерозом (КаА) в сравнении со здоровыми донорами крови.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование было включено 264 субъекта: 88 пациентов с гемодинамически значимым (потребовавшим проведения каротидной эндартерэктомии вследствие ишемического инсульта (n=44) или хронической ишемии головного мозга (n=44)) КаА, 88 пациентов с гемодинамически значимым (ставшим причиной стабильной ишемической болезни сердца или инфаркта миокарда (ИМ)) КоА и 88 участников эпидемиологического исследования PURE (Prospective Urban Rural Epidemiology Study), не имеющих гемодинамически значимого КаА по данным ультразвукового исследования и симптоматического КоА. У включенных в исследование пациентов определялись параметры системного минерального гомеостаза: уровень общего и ионизированного кальция, фосфора, а также концентрации общего белка и ингибиторов внескелетной кальцификации альбумина и фетуина-А в сыворотке крови. Помимо этого, посредством перенасыщения (+2 ммоль/л) сыворотки крови пациентов солями кальция (CaCl2) и фосфора (Na2HPO4) с последующей инкубацией при 37° С в течение 24 ч определялась ее склонность к формированию КФБ в сравнении с контрольной сывороткой.</p></sec><sec><title>Результаты</title><p>Результаты. Выявлено, что пациенты с КаА (как нестабильным, т. е. вызвавшим ишемический инсульт, так и стабильным) и ИМ характеризуются повышенной склонностью сыворотки крови к формированию КФБ, а также сниженной концентрацией общего белка и альбумина в сочетании с повышенным уровнем ионизированного кальция. Корреляционный анализ позволил выявить, что формирование КФБ в сыворотке крови ассоциировано со снижением общего белка и альбумина. Таким образом, при истощении белкового депо в крови повышается уровень свободных ионов кальция, нейтрализуемых далее путем формирования КФБ, являющихся вторичным депо ионов кальция.</p></sec><sec><title>Заключение</title><p>Заключение. Формирование КФБ в сыворотке крови обусловлено вызванным снижением концентрации общего белка и альбумина, повышением уровня ионизированного кальция и ассоциировано с развитием атеросклероза в различных сосудистых бассейнах, включая прогрессирование КоА от стабильной ишемической болезни сердца до ИМ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the formation of calcium phosphate bions (CPB), a trigger of endothelial dysfunction in the patients with coronary and carotid atherosclerosis as compared to healthy blood donors.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 264 individuals: 88 patients with cerebrovascular disease who underwent carotid endarterectomy due to ischemic stroke (n=44) or chronic cerebral ischemia (n=44)); 88 patients with coronary artery disease (44 patients with stable angina requiring coronary artery bypass graft surgery and 44 patients with myocardial infarction); 88 healthy volunteers enrolled into Prospective Urban Rural Epidemiology (PURE) study who did not have symptomatic coronary or hemodynamically relevant carotid atherosclerosis as measured by ultrasound. We further measured serum parameters defining mineral homeostasis: total and ionized calcium, phosphate, total protein, albumin, and fetuin-A. We also determined serum calcification propensity by means of supersaturation (+2 mmol/L) with CaCl2 and Na2HPO4 followed by incubation at 37° С for 24 hours.</p></sec><sec><title>Results</title><p>Results. Patients with carotid atherosclerosis (either with ischemic stroke or chronic cerebral ischemia) or myocardial infarction were characterized by an increased calcification propensity in combination with reduced total protein and albumin and elevated ionized calcium. Correlation analysis revealed that serum calcification was associated with a decrease in serum total protein and albumin. Therefore, depletion of serum protein mineral depot leads to the elevation of ionized calcium which is further aggregated into calcium phosphate bions, a secondary mineral depot.</p></sec><sec><title>Conclusion</title><p>Conclusion.
