Изучить, как влияет терапия препаратами из групп ингибиторов дипептидилпептидазы-4 (иДПП-4) и натрий-глюкозного котранспортера-2 (иНГЛТ-2) на показатели сосудистой жесткости у пациентов с сахарным диабетом 2 типа (СД2) и высоким сердечно-сосудистым риском.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведено открытое проспективное 24-недельное исследование, в которое были включены 120 пациентов с СД2 и высоким сердечно-сосудистым риском. У них оценивали влияние терапии представителями современных классов сахароснижающих препаратов — иНГЛТ-2 (эмпаглифлозин 25 мг/сут.) и иДПП-4 (ситаглиптин 100 мг/сут.) на сосудистую эластичность, параметры центральной гемодинамики, лабораторные показатели.</p></sec><sec><title>Результаты</title><p>Результаты. На фоне 24 недель терапии целевого уровня гликированного гемоглобина достиг 71% пациентов в группе ситаглиптина и 80% — в группе эмпаглифлозина. В обеих группах улучшились параметры сосудистой жесткости и центральной гемодинамики, однако достоверные изменения показателей зарегистрированы только в группе пациентов, принимавших эмпаглифлозин (скорость распространения пульсовой волны на каротидно-феморальном участке снизилась на 14,4%, индекс аугментации — на 6%, центральное пульсовое давление  — на  7,8%) (p&lt;0,05). Прием ситаглиптина был ассоциирован с достоверными улучшениями липидного профиля пациентов (снижение общего холестерина на 9,5%, триглицеридов на 21%, липопротеидов низкой плотности на 15,1%, увеличение уровня липопротеидов высокой плотности на 15,7%) (p&lt;0,05). В группе эмпаглифлозина улучшились антропометрические показатели (индекс массы тела снизился на 9,1%, окружность талии — на 4,1%) (p&lt;0,05). Пациенты обеих групп продемонстрировали статистически значимое снижение индекса инсулинорезистентности HOMA и высокочув-ствительного С-реактивного белка: на 34% и 51,6% в группе эмпаглифлозина, на  31,8% и  22,1% в  группе ситаглиптина, соответственно (во всех случаях  p&lt;0,05).</p></sec><sec><title>Заключение</title><p>Заключение.Применение эмпаглифлозина в большей степени связано со снижением артериальной ригидности у пациентов с  СД2 и  высоким сердечно-сосудистым риском, по  сравнению с  использованием ситаглиптина.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the effect of various classes of glucose-lowering agents (dipeptidyl peptidase-4 (DPP-4) and sodium-glucose cotransporter-2 (SGLT-2) inhibitors) on the vascular stiffness in patients with type 2 diabetes (T2D) and high cardiovascular risk.</p></sec><sec><title>Material and methods</title><p>Material and methods. The open-label, prospective 24-week study included 120 patients with T2D and high cardiovascular risk. We evaluated the effect of modern glucose-lowering medications, empagliflozin at a dose of 25 mg/day (SGLT-2 inhibitor) and sitagliptin at a dose of 100 mg/day (DPP-4 inhibitor), on vascular elasticity, central hemodynamic and laboratory parameters.</p></sec><sec><title>Results</title><p>Results. After 24-week therapy, the target glycated hemoglobin level reached 71% and 80% of patients in the sitagliptin and empagliflozin groups, respectively. In both groups, vascular stiffness and central hemodynamic parameters were improved. However, significant changes were recorded only in the empagliflozin (carotid-femoral pulse wave velocity decreased by 14,4%, augmentation index — by 6%, central pulse pressure — by 7,8%) (p&lt;0,05). Use of sitagliptin was associated with significant improvements in the lipid profile (total cholesterol decreased by 9,5%, triglycerides — by 21%, low density lipoproteins — by 15,1%; high density lipoproteins increased by 15,7%) (p&lt;0,05). In the empagliflozin group, anthropometric parameters were improved (body mass index decreased by 9,1%, waist circumference — by 4,1%) (p&lt;0,05). Patients in both groups had a significant decrease in HOMA-IR index and highly sensitive C-reactive protein: by 34% and 51,6% in the empagliflozin group and by 31,8% and 22,1% in the sitagliptin group, respectively (p&lt;0,05).</p></sec><sec><title>Conclusion</title><p>Conclusion.
