Выявление ассоциации между наличием полиморфизма гена CYP2С19*2 и повторным ранним тромбозом коронарного стента, а также парадоксальным лабораторным ответом на прием клопидогреля у пациентов — жителей Сибирского региона — после острого коронарного синдрома. Материал и методы. Обследовано 105 человек, госпитализированных для проведения стентирования коронарных сосудов по поводу острого коронарного синдрома, не получавших ранее клопидогрель. Проводилась оценка полиморфизмов гена СУР2С19: *2, *3, *17 аллелей, оценка агрегации тромбоцитов с АДФ до и после приема клопидогреля, конечных точек безопасности и эффективности в течение 30 дней (тромботические осложнения, кровотечения).Результаты. На исследованной выборке достоверной ассоциации между носительством хотя бы одного аллеля CYP2С19*2 и/или CYP2С19*3 и лабораторной парадоксальной реакцией не обнаружено. Была обнаружена достоверная ассоциация между носительством аллеля CYP2C19*17 и наличием кровотечений. При сравнении групп пациентов, имеющих и не имеющих осложнения, связанные с приемом клопидогреля (как тромботических, так и кровотечений), обнаружено достоверное различие по показателю остаточной агрегации тромбоцитов.Заключение. Полученные результаты могут быть крайне важны для принятия решений о стратегии двойной антиагрегантной терапии и тактике выбора лекарственных препаратов.</p></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To reveal the association of gene polymorphism CYP2С19*2 and recurrent early stent thrombosis in coronary vessels, and paradoxal response to clopidogrel intake in patients — the inhabitants of Siberian Region — after acute coronary syndrome.</p></sec><sec><title>Material and methods</title><p>Material and methods. Totally 105 patients studied, hospitalized for stenting of coronary arteries in acute coronary syndrome, never received clopidogrel previously. The polymorphism of studied СУР2С19: *2, *3, *17 alleles, as assessment of platelet aggregation with ADP before and after clopidogrel intake, as of endpoints on safety and efficacy during 30 days (thrombotic complications, bleeding).</p></sec><sec><title>Results</title><p>Results. Within the selected patients there was no any significant association of any CYP2С19*2 and/or CYP2С19*3 alleles and paradoxic laboratory reaction. There was significant association of the CYP2C19*17 gene carriage and bleedings. When comparing the groups of patients having or not having complications related to clopidogrel (thrombotic or bleedings), there was significant difference in residual platelet aggregation.</p></sec><sec><title>Conclusion</title><p>Conclusion.
