В статье сообщается о первом в России опыте транскатетерной имплантации по методу “клапан-в-клапан” (TVIV) у пациентов с тяжелым трикуспидальным стенозом (ТС) вследствие структурной дегенерации хирургического биопротеза трикуспидального клапана (ТК).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. TVIV выполнена 4 пациентам разных возрастных групп (от 18 до 68 лет) высокого хирургического риска (от 1 до 3 стернотомий в анамнезе), со структурной дегенерацией биопротеза ТК.</p></sec><sec><title>Результаты</title><p>Результаты. Технический успех был достигнут в 100% случаев. В послеоперационном периоде у всех пациентов наблюдали нормализацию диастолических градиентов на ТК. Средняя величина пикового диастолического градиента снизилась с 20,4 мм рт.ст. до 10 мм рт.ст. У 3 пациентов с застойной недостаточностью кровообращения после TVIV отмечено клиническое улучшение в виде увеличения переносимости физической нагрузки по результатам теста с 6-минутной ходьбой. У 1 асимптомного пациента c признаками правожелудочковой дисфункции наблюдалась нормализация эхокардиографических показателей.</p></sec><sec><title>Заключение</title><p>Заключение. TVIV представляет собой малоинвазивную альтернативу традиционному хирургическому репротезированию ТК. Результаты имеющихся к настоящему времени исследований, включая собственный опыт, свидетельствуют о том, что TVIV является эффективным и безопасным методом коррекции дисфункции биопротеза ТК у пациентов высокого риска различной возрастной категории. Для оценки долгосрочных результатов метода необходимы дальнейшие исследования.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. In the issue we report first in Russia experience of transcatheter “valve-invalve” implantation (TVIV) for treatment of severe tricuspid stenosis due to the structural deterioration of surgical tricuspid bioprosthesis.</p></sec><sec><title>Material and methods</title><p>Material and methods. TVIV was performed in 4 high-risk redo patients (1 to 3 previous sternotomies) of various ages across (18-68 years) with structural deterioration of surgical tricuspid bioprosthesis.</p></sec><sec><title>Results</title><p>Results. Technical success was achieved in 100% cases. Diastolic gradients on tricuspid valve markedly decreased in all patients. Peak transtricuspidal gradient decreased from 20,4 to 10 mmHg in average. Clinical improvement as assessed by 6-minute walk test after TVIV was observed in 3 patients with congestive heart failure. In 1 patient with asymptomatic right ventricle dysfunction TVIV resulted in the enhancement of echocardiographic parameters.</p></sec><sec><title>Conclusion</title><p>Conclusion. TVIV is a mininvasive alternative to conventional surgical tricuspid valve redo replacement. Based on available data including own experience TVIV should be considered an effective and safe treatment option for failed TV bioprostheses in high-risk patients of different age.
