Оценить наколенный опыт ФГБУ НМИЦ им. акад. Е. Н. Мешалкина и НИИ кардиологии Томского НИМЦ, г. Томск в хирургическом лечении расслоений аорты I типа по Де Бейки с помощью гибридных методик (“замороженный хобот слона”) по  сравнению с  классическими стандартными вмешательствами.</p></sec><sec><title>Материал и методы</title><p> Материал и методы. В исследование включены пациенты, оперированные последовательно в период с 2001 по 2017гг с диагнозом: острое и подострое расслоение аорты I типа по Де Бейки. Выполнено ретроспективное обсервационное сравнение результатов хирургического лечения двух групп пациентов: с применением стандартных хирургических подходов (косой агрессивный анастомоз или полное протезирование дуги аорты многобраншевым протезом) и вмешательство по типу “замороженного хобота слона” (frozen elephant trunk — FET). В группы вошли 70 и 31 пациент, соответственно. С целью повышения достоверности результатов по методике propensity score match. Оценивались летальность и  заболеваемость в послеоперационном периоде, частота развития аорто­связанных событий за период наблюдения.</p></sec><sec><title>Результаты</title><p> Результаты. Летальность и частота осложнений между группами в раннем и отдаленном послеоперационном периодах не отличалась. При оценке аортосвязанных событий в  отдаленном периоде группы статистически значимо различались в пользу гибридных вмешательств (p=0,005).</p></sec><sec><title>Заключение</title><p>Заключение. Гибридная методика реконструкции грудного отдела аорты по типу “замороженного хобота слона” позволяет выполнить одномоментную реконструкцию аорты при ее “многоэтажном” поражении. Наблюдение пациентов в отдаленном периоде позволяет заявить о длительном, стабильном результате хирургического лечения при отсутствии дополнительных периоперационных рисков.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To assess results of Meshalkin National Research Medical Center and Tomsk National Research Medical Center in surgical treatment of DeBakey type I aortic dissection using hybrid technologies (“frozen elephant trunk”) compared with classical standard interventions.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included patients operated on successively in the 2002­2010 timeframe with a diagnosis of acute and subacute DeBakey type I aortic dissection. A retrospective observational comparison of the surgical treatment results of patients (two groups) was performed: using standard surgical approaches (beveled anastomosis or aortic arch replacement using multi­branched prosthesis) and frozen elephant trunk (FET) intervention. The groups included 70 and 31 patients, respectively. Mortality and morbidity were evaluated in the postoperative period, the frequency of aorto­related events — during the observation period.</p></sec><sec><title>Results</title><p>Results. Mortality and the frequency of complications between the groups in the early and late postoperative periods did not differ. Aorto­related events of the groups in the long­term period differed statistically significantly in favor of hybrid interventions (p=0,005).</p></sec><sec><title>Conclusion</title><p>Conclusion. The hybrid technology of thoracic aorta reconstruction using “frozen elephant trunk” type allows to make one­step reconstruction of the aorta during its multilevel lesion.
