Провести анализ результатов применения в практике врача-кардиолога опросника Европейского общества кардиологов (ЕОК) по оценке комплекса неконвенционных факторов риска у больных с ишемической болезнью сердца (ИБС) в условиях стационара.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. У 310 больных с ИБС (средний возраст 59,6±8,8 лет, мужчин 62,2%), подписавших информированное согласие на участие в исследовании, проведено общеклиническое обследование с выявлением конвенционных факторов риска, анализом достижения стандартных целей лечения. Врачом-кардиологом выполнена двухэтапная оценка неконвенционных факторов риска (НФР) с применением анкеты Европейского общества кардиологов (ЕОК) и валидизированных опросников (DS-14, шкалы депрессии и тревоги Гамильтона) для клинической идентификации тревоги, депрессии, типа личности Д.</p></sec><sec><title>Результаты</title><p>Результаты. У госпитализированных пациентов отмечались преимущественно тяжелые клинические проявления заболевания: острый коронарный синдром (51,6%), снижение фракции выброса менее 50% по данным эхокардиографии (40%), острый инфаркт миокарда в анамнезе (29%), а также высокая частота конвенционных факторов риска: артериальной гипертензии (75,8%), дислипидемии (75,1%), ожирения (40,9%). Большинство респондентов не достигли стандартных целей лечения: 13,8% больных курили, 30,9% не соблюдали гиполипидемическую диету, 81,3% не употребляли рыбу в рекомендуемом количестве, 51,6% не выполняли режим двигательной активности, 40,6% не достигли целевого уровня артериального давления, 59,3% — целевого уровня липопротеидов низкой плотности, 59,8% — целевого уровня индекса массы тела и нуждались в оценке факторов, препятствующих эффективному лечению. В комплексе выявленных НФР превалировали враждебность (30,9%), тип личности Д (25,4%), низкий социально-экономический статус (12,2%) и тревога (10%), реже встречались депрессия (5,1%), стресс на работе и в семейной жизни (4,5%) и социальная изоляция (1,6%).</p></sec><sec><title>Заключение</title><p>Заключение. Результаты по частоте встречаемости НФР согласуются с данными, полученными зарубежными исследователями. Применение опросника ЕОК позволяет ориентировать практикующих врачей на выявление НФР, дополнить данные об индивидуальном профиле риска и расширить спектр рекомендаций по лечению и профилактике у больных с ИБС.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To analyze the results of using of the European Society of Cardiology (ESC) questionnaire regarding the assessment of non-conventional risk factors in patients with coronary artery disease (CAD).</p></sec><sec><title>Material and methods</title><p>Material and methods. Three hundred and ten patients with CAD (mean age 59,6±8,8 years, men 62,2) underwent a general clinical examination with the identification of conventional risk factors and assessing standard treatment goals. The cardiologist performed a two-stage assessment of non-conventional risk factors using the ESC questionnaire and validated questionnaires (DS-14, Hamilton Rating Scale for Depression and Anxiety) for clinical identification of anxiety, depression, type D personality.</p></sec><sec><title>Results</title><p>Results. In hospitalized patients, predominantly we noted severe clinical manifestations of the disease: acute coronary syndrome (51,6%), a decrease in the ejection fraction of less than 50% (40%), a history of acute myocardial infarction (29%), and a high frequency of conventional risk factors: arterial hypertension (75,8%), dyslipidemia (75,1%), obesity (40,9%). The majority of respondents did not reach the standard treatment goals: 13,8% of patients smoked, 30,9% did not follow the lipid-lowering diet, 81,3% did not follow recommendations regarding fish eating, 51,6% did not have recommended physical activity, 40,6% did not reached the target level of blood pressure, 59,3% — the target level of low-density lipoproteins, 59,8% — the target level of body mass index. We determined prevalence of following non-conventional risk factors: hostility (30,9%), type D personality (25,4%), low socio-economic status (12,2%) and anxiety (10%). Depression (5,1%), stress at work and in family life (4,5%), social exclusion (1,6%) were less common.</p></sec><sec><title>Conclusion</title><p>Conclusion. The results are consistent with data obtained by foreign researchers.
