На сегодняшний день многое известно о различных шкалах прогнозирования риска развития инфаркта миокарда и смерти в периоперационном периоде, а также о медикаментозных возможностях их профилактики. Вопрос о пользе выполнения коронарографии и превентивной коронарной реваскуляризации перед внесердечными хирургическими вмешательствами у пациентов высокого кардиального риска остается спорным. В данном обзоре рассмотрены взгляды на патогенез развития периоперационного инфаркта миокарда, а также подробно проанализированы результаты существующих ретроспективных и проспективных исследований эффективности и безопасности различных видов предварительной коронарной реваскуляризации для снижения частоты кардиальных осложнений внесердечных хирургических вмешательств.</p></abstract><trans-abstract xml:lang="en"><p>Prognosis and prevention of cardiac complications of non-cardiac surgery is an important clinical task. Currently quite a lot is known on various scales of myocardial infarction risk prognosis and the death in perioperational period, as on drug treatment and prevention. The issue whether to perform coronary arteriography and preventive coronary revascularization before noncardiac surgical interventions in patients with high cardiological risk score remains controversial.
