Выявление наиболее информативных показателей углеводного и липидного обмена, ассоциированных с развитием сахарного диабета вто­рого типа через год после перенесенного инфаркта миокарда. Материал и методы. Обследовано 200 пациентов с инфарктом миокарда (ИМ), у которых на 1-е и 12-е сутки госпитализации определяли содержание глюкозы, инсулина, индекс инсулинорезистентности (ИР), показатели липид­ного профиля. В течение года после перенесенного заболевания оценивали новые случаи манифестации сахарного диабета второго типа (СД). Результаты. Через год после перенесенного ИМ СД второго типа был диагно­стирован у 14,5% пациентов и ассоциировался с наличием в анамнезе факто­ров сердечно-сосудистого риска, неблагоприятным клиническим течением заболевания в острый и отдаленный периоды, нарушением липидного обмена. Высокий риск развития СД в отдаленном периоде ИМ связан с наличием ИР и возрастанием в 9,5 раз концентрации свободных жирных кислот (СЖК) в плазме крови в остром периоде заболевания.</p></sec><sec><title>Заключение</title><p>Заключение. Значимыми факторами риска развития СД через год после перенесенного ИМ, наряду с ИР, характеризующейся гипергликемией и гипер- инсулинемией, является увеличение концентрации СЖК в госпитальном пери­оде заболевания. СЖК являются перспективными маркерами для стратифика­ции риска развития СД у больных с ИМ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To identify the most informative parameters of carbohydrate and lipid metabolism which predict the manifestation of type 2 diabetes mellitus (DM-2) within one year after myocardial infarction (MI).</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 200 MI patients who underwent the assessment of glucose and insulin levels, insulin resistance (IR) index, and lipid profile at Days 1 and 12. The incidence of new DM-2 cases which manifested within a year after MI was assessed.</p></sec><sec><title>Results</title><p>Results. After one year after MI, incident DM-2 was diagnosed in 14,5% of the patients. It was associated with concomitant cardiovascular risk factors, adverse clinical course of acute and longer-term MI periods, and lipid metabolism disturbances.
